GP Management Plan
A GP Management Plan (GPMP) is a structured, written plan for those suffering with a chronic medical condition that details your management goals, the actions to be taken by you, the actions to be taken by your health care providers and the treatments and services you are likely to need.
A GPMP has several purposes:
- It serves as a summary of your condition.
- It acts as an organisational tool for your doctor.
- If another doctor becomes involved it can provide important information.
Anyone who has an ongoing medical condition lasting longer than six months (e.g. heart disease, diabetes, arthritis) may benefit from a GPMP. People who require palliative care services or people being discharged from hospital may also benefit. There are no age restrictions.
