GP Chronic Disease Management Plans
Do you have a chronic health condition such as asthma, emphysema, osteoarthritis, diabetes or heart disease?
Please book in with our nurses to arrange a time for a review of your health.
Through the care plan system you may be eligible for Medicare funded allied health care such as physiotherapy, podiatry or dietetics.
Home Medicines Review
A Home Medicines Review is a comprehensive clinical review of your medicines in your home by an accredited pharmacist on referral from your GP.
A Home Medicines Review can help you to gain knowledge and understanding of the medicines you are using and how they work.
With the help of your GP and pharmacist, you can become an active participant in the decisions that support your healthcare plan.
The purpose of a health assessment is to help identify any risk factors that may require further health management.
In addition to assessing your health status, a health assessment is used to identify a broad range of factors that influence your physical, psychological and social functioning.
