Comprehensive Health Assessments are provided by our practice.
A Health Assessment provides a structured way of identifying health issues and conditions that are potentially preventable or amenable in order to improve your health and/or quality of life.
This scheme is completely funded by the Government through Medicare, and aims to provide a review of a patient's current health needs. This then enables our patients stay in their own homes for as long as possible and to assist them in staying safe, well and happy.
This appointment is held at the surgery, takes approximately an hour, and this service will be provided at no out of pocket cost to you – it will be bulk-billed to Medicare.
Firstly, one of our practice nurses will assess your overall health and well being. They are then also able to provide information about such resources as handrails, Meals on Wheels and cleaning, and also local, community, council and medical services such as the Department of Veteran’s Affairs and the Royal District Nursing Service should you require them.
Immediately following your consultation with our practice nurse, your doctor will see you to review and discuss the assessment and to answer any questions you may have.
