What is a care plan?
A GP Management Plan (GPMP) is a structured, written plan for those suffering with a chronic medical condition that details your management goals, the actions to be taken by you, the actions to be taken by your health care providers and the treatments and services you are likely to need.
A GPMP has several purposes:
It serves as a summary of your condition.
It acts as an organisational tool for your doctor.
If another doctor becomes involved it can provide important information.
Anyone who has an ongoing medical condition lasting longer than six months (e.g. heart disease, diabetes, arthritis) may benefit from a GPMP. People who require palliative care services or people being discharged from hospital may also benefit. There are no age restrictions.
What is a health assessment?
Arkaba Medical Centre offers patients who are over the age of 75 the opportunity to have an annual Health Assessment to help maintain good health.
The health assessment is an opportunity for your doctor to complete a thorough check-up of your medical, physical, psychological and social health in a structured way. This is also an opportunity for you to raise any concerns that you may have relating to your health with your doctor and nursing team.
Our knowledgeable nursing staff may be able to advise and guide you in obtaining home care services, which may assist you in staying in your own home for longer. For example, Meals on Wheels, and having your house fitted for handrails.
